PGP-MP Principle - Dr Elian Perceli (GP)
All GPs are aware that patients often attend their 10-20 minute appointments with a list of issues to discuss. When reviewing a patient’s medical records, I sometimes find additional concerns that also require attention and cannot wait for another appointment. However, the short appointment time makes it difficult to cover everything.
To provide the best care for my patients while considering the resources of both myself and the staff, I sometimes decide to address multiple medical problems in one appointment instead of advising them to book a separate appointment. The patient needs to understand what we discuss and retain sufficient information to follow our plan.
Patients may need to remember a lot of information, which can make it hard for them to make quick decisions on unfamiliar topics. It is also crucial to accurately document the consultation details.
To help with these challenges, I use the "Patient GP Management Plan" (PGPMP). This plan outlines the agreed-upon plan. The Management Plan typically includes explanations and details about the medical plan we discussed.
I give the PGPMP to the patient (either a printed form or an email) for their reference.
