How I Create a Medical Plan (PGP-MP)
The essence of creating a PGP-MP lies in having a set of templates that I can copy and paste into the plan. These templates cover frequently used medications, investigations, treatment schemes, and instructions for monitoring different treatments or disease progression. Each clinician can and should create their own templates or adapt existing templates that include the information they deem appropriate for patients.

It is important to emphasise that the templates I use for creating PGP-MP are customised and modified as needed for each patient and consultation. I want to clarify that I am not promoting any of my templates. Each general practitioner (GP) should develop their own templates, and it is their sole responsibility to use them appropriately in communication with their patients.
All templates on this website are for demonstration purposes only. Each user is responsible for deciding whether to use them, and I do not encourage their use.
Here, I will describe how I practically create a PGP-MP. You can also watch videos on creating a PGP-MP.
There are three main ways to create an MP:
1. Using an MP "Word" working page
2. Using an MP "PDF" working page
3. Creating an MP using clinical speech recognition software
I am sure that anyone can create or learn how to create the Word and PDF working pages.
a) Using a PGP-MP "Word" Working Page
I created a Word document that contains two tables. At the top of the document, a table with the alphabet is provided. Each letter in the alphabet table links to the second table, which is organised alphabetically by titles. These titles link to subjects that contain the information I want the patient to remember, making it easy for me to copy and paste the appropriate text into the PGP-MP.
I recommend using two monitors for this process: one for the patient's Medical Record (MR) and the other for the PGP-MP "Word" working page. You can refer to an example Word working page available for download to see how it works without further explanation.
b) Using a PGP-MP "PDF" Working Page
Creating a PGP-MP with a PDF working page is similar to using a Word document, with some key differences:
- Whilst using a PDF document, the links can be opened by left-clicking on them, rather than by holding the "Ctrl" key and left-clicking.
- Unlike in Word, you cannot edit the text in a PDF.

c) Creating an MP Using Clinical Speech Recognition Software
When creating a PGP-MP using speech recognition software, I utilise the same information recorded in the MP "Word" or "PDF" working pages. The clinical speech recognition software includes a feature called "Auto-text or snipped-text," which allows me to insert specific text into a document using voice commands.
I have set up "Auto-text commands" for all the templates and subjects recorded in the Word and PDF working pages. This means that when I use clinical speech recognition software, I no longer need to search for or click on links to access the templates. For example, if I want to include information about QRISK in the MP, I simply say "explanation QRISK," and the software automatically inserts the relevant text.
